Bolstered by billions in federal funding tied to sweeping legislative changes, states like Louisiana are launching venture capital-style tech catalyst funds. These initiatives aim to bypass decades of declining infrastructure and hospital closures by injecting capital directly into emerging health technology enterprises.
The influx of capital stems from the federal Rural Health Transformation Program, a $50 billion initiative established by lawmakers to mitigate the impact of more than $900 billion in projected Medicaid spending reductions over a decade, resulting from major tax and spending legislation enacted in 2025. Rather than serving purely as a budget stabilization tool, the program explicitly directs states to pioneer innovative approaches for revitalizing medically underserved rural regions facing severe physician shortages and shrinking medical facilities.
For state economic development offices accustomed to funding traditional infrastructure or software startups, the infusion of federal money represents a profound shift. Josh Fleig, chief innovation officer for the Louisiana Economic Development agency, expressed astonishment upon learning his state had secured $20 million annually over five years to invest explicitly in rural health startups.
Louisiana, frequently ranked by public health metrics as one of the least healthy states in the nation—with disproportionately high rates of obesity, diabetes, and cardiovascular disease—received $208.4 million for the inaugural year of the rural health program. With approximately 1.1 million residents living in rural parishes, state officials view this capital as a transformative opportunity to test cutting-edge technological solutions in an environment where they are most urgently needed.
The Rise of State-Level Tech Catalyst Funds
Modernizing technology infrastructure is a foundational pillar of the federal rural health framework. The program encourages a “move fast and innovate quickly” philosophy reminiscent of Silicon Valley’s venture capital ecosystem, albeit with a public service mandate. Aaron Bujnowski, a managing director with the healthcare industry group at the consultancy Alvarez & Marsal, noted that the objective is to accelerate innovation, embrace rapid prototyping and failure, and swiftly transition successful models to long-term operational sustainability.
In addition to Louisiana, at least seven other states—including Delaware, Georgia, Massachusetts, Nebraska, South Carolina, Virginia, and West Virginia—have established dedicated rural health tech catalyst funds, according to Centers for Medicare & Medicaid Services (CMS) spokesperson Timothy Foster.
The distribution of the federal awards varies significantly by state, reflecting diverse geographic and demographic challenges. First-year allocations range from $147 million in New Jersey to $281 million in Texas. Under federal guidelines, states are permitted to allocate up to 10% of their total program awards toward establishing and maintaining these specialized technology catalyst funds.
This sudden influx of venture capital has drawn immediate interest from early-stage companies seeking deployment opportunities. In Louisiana, state officials introduced their tech catalyst fund during an event in Natchitoches, drawing applications from more than 200 companies competing for seed investments ranging from $250,000 to $3 million.
Among the applicants is Greens Health, a two-year-old startup that analyzes Medicare claims data to identify patients suffering from chronic illnesses such as diabetes, subsequently coordinating care with local home health providers and senior facilities. Kehlin Swain, co-founder and chief executive of Greens Health, noted that while the company currently serves approximately 100 patients across Texas, Alabama, and Florida, securing a $250,000 investment from Louisiana would provide a vital pathway for regional expansion.

Similarly, Caret Health, a four-year-old enterprise founded by physician-researcher Riya Pulicharam and engineer Kevin Zhao, utilizes a proprietary technology platform to identify patients needing assistance with medical appointments, diagnostic scans, and prescription pickups, subsequently triggering automated communications and human follow-ups. Co-founder Kevin Zhao explained that while scaling operations within established, vendor-heavy major health systems proved challenging, the paucity of existing infrastructure in rural areas has allowed the company to integrate rapidly and meet critical hospital demands. Caret Health currently contracts with approximately 60 hospitals across 16 states and is pursuing a $3 million investment to expand its footprint into Louisiana.
Strict Federal Oversight, Deadlines, and Accountability
The rapid deployment of public venture capital is governed by rigorous regulatory frameworks and compressed timelines overseen by CMS. States must compete annually to maintain their funding allocations within the five-year federal program. Federal regulators retain the authority to reallocate funds away from states that fail to meet promised benchmarks, including specific targets for disbursing capital to technology innovators.
CMS has established a stringent seven-step guidance framework for administering the tech catalyst funds. To ensure transparency and compliance, states are required to submit a comprehensive list of funding finalists to CMS at least 15 business days prior to public announcements, accompanied by sufficient documentation for federal officials to evaluate each proposed project. Furthermore, participating startups must meet specific eligibility criteria: they must be less than 10 years old and have raised under $50 million in prior early-stage funding. Disbursal of funds is strictly contingent upon meeting predetermined developmental milestones, backed by targeted federal audits as necessary.
A critical milestone occurred at the end of August, marking the deadline for states to submit their initial annual progress reports to CMS. Although CMS has chosen not to publicly release these individual state reports—opting instead to publish a comprehensive annual progress summary—states face an even more immediate deadline. Under federal guidelines, first-year funds must be legally obligated by October 30 to secure continued financing for the second year of the program.
Daniel X. O’Neil, an open-data and open-government technology consultant who developed an independent state tracking resource for the Rural Health Transformation Program, has closely monitored the application and reporting phases. Observing the strict procedural requirements and looming deadlines, O’Neil highlighted the high stakes of the upcoming federal evaluations, pointing to the likelihood of funding clawbacks for non-compliant jurisdictions.
Balancing Innovation with Patient Protection and Oversight
While federal guidelines mandate compliance with established standards regarding data privacy, security, interoperability, and patient safety, policy analysts have raised concerns regarding the specific protections embedded within the catalyst fund framework. According to Josh Fleig, this model is designed to create a self-sustaining financial cycle. Conversely, if certain startups fail—a standard outcome in early-stage venture investing—the state relies on the law of averages across its diversified portfolio to maintain financial viability.
"If we are good, we’ll make more money than we spent," Fleig stated. "Either way, it’s going to go back into improving healthcare outcomes."
